Day Rehabilitation
Our day programs are suitable for people who are well enough to attend for 3-4 hours, one to two times a week. This includes patients who require ongoing rehab after hospital discharge as well as members of the community who need allied health support after injury or illness.
Most Private Health Funds cover the cost of Day Rehabilitation.
Your Rehabilitation Team
Southern Highlands Private Hospital supports a multidisciplinary approach to health care to achieve the best possible outcome.
The team includes:
- Rehabilitation Specialist
- Rehabilitation Nurses
- Physiotherapists
- Occupational Therapists
Additional Support as required:
- Speech Therapists
- Social Worker
- Discharge Planner
- Dietitian
For referrals to our Day Rehabilitation Program, the referring hospital or GP MUST provide the following documentation:
- Doctor Referral Letter
- Completed Referral RHC 45 – Rehabilitation Unit Pre-Admission and Referral Form
- Operation Report
- Hospital/Physiotherapy/Occupational Therapy Discharge Summary
- EMU Agreement (if applicable)
- MRSA Status
We cannot proceed with accepting a patient onto the program without the above documentation, as it is a requirement from the health funds.
