How to Refer

The patient should present at the hospital to complete an Admission Form with additional personal information or they can complete an On-Line Admission Form.

Surgical patients are referred to the hospital when the Surgeon completes a Booking Form. The patient should present at the hospital to complete additional personal information. The majority of surgical patients will be asked to attend a pre-admission clinic. Alternatively, a theatre time is arranged directly with the Operating Theatre Manager.
